The table below provides summaries of infection prevention and control (IPAC) lapses that have been identified at local premises. These lapses were found during investigations prompted by complaints, referrals, or communicable disease surveillance.

Please note:

  • Only premises where a lapse was confirmed are listed

  • Premises that were investigated but found to have no IPAC lapse are not included

  • These reports reflect findings at a specific point in time.

  • They do not guarantee that listed or unlisted premises are currently free of IPAC concerns

  • All reports listed here are for premises located in Oxford County, Elgin County, or the City of St. Thomas

  • Each report corresponds to a single location
